Consent and Release of Liability for Reiki & Bodywork

I understand that Reiki is a relaxation and stress reduction technique. I acknowledge that Reiki and bodywork treatments administered are only for the purpose of helping me relax and to relieve stress. Reiki and bodywork practitioners do not diagnose conditions, nor do they prescribe substances or perform medical treatment, nor interfere with the treatment of a licensed medical professional. I further understand that Reiki and bodywork received from practitioners are not to be construed as a substitute for medical examination, diagnosis or treatment. It is recommended that I see a licensed health care professional for any physical or psychological ailment I may have. I also understand that Reiki, an energetic healing methodology and bodywork involves the laying on of hands. I understand that I will be fully clothed during the session.

Having read, completed and understood the foregoing, I request to receive Reiki and/or bodywork. I understand that my practitioners are providing Reiki and/or bodywork for me at my request, and are not responsible for the outcome of the session. I release Be You Thank You, LLC (DBA Jeana Johnson) and other practitioners during events from any liability, including intended/unintended results or injury resulting from treatments, tools and/or techniques.
